Youth Ministry allows you to download and open up permission slips on line. 

Some Permission Slips require Adobe Acrobat Reader.  This software comes already          installed on most computers.  If you are having trouble opening up a permission slip then     you probably need Adobe Acrobat Reader. Click on the link  below to get your free copy   of Adobe Reader.

All Permission slips are also available at the Parish Office

Remember, all permission slips must be returned to the Parish Office by the deadline.
